Please note, we cannot take adult referrals from third parties via this form, referrals can only be made by the adult themselves. If there are specific reasons you feel a third-party referral is appropriate and you have full consent of the adult involved, then please call our administration team.

Please note we are not a crisis service. If you find yourself at the point of crisis, please call your GP and request an urgent appointment (during surgery hours) or take yourself to your local accident and emergency (A&E) department.
